Our Flu clinics are starting from Wednesday 21st September 2016. We have a special flu clinic on Saturday 24th September 2016.

It is highly recommended by NHS that those listed below have a flu jab.
If you are eligible to have a flu jab but you decide not to have it this year then please inform us in order for us to update your records so we do not contact you regarding your flu jab due.
Recommended list of people
everyone aged 65 and over • everyone under 65 years of age who has a medical condition listed below, including children and babies over six months of age • all pregnant women, at any stage of pregnancy • all two, three and four year-old children • all children in school years 1, 2 and 3 • all primary school-aged children in some parts of the country • everyone living in a residential or nursing home • registered carers
a heart problem • a chest complaint or breathing difficulties, including bronchitis, emphysema or severe asthma • a kidney disease • lowered immunity due to disease or treatment (such as steroid medication or cancer treatment) • liver disease • had a stroke or a transient ischaemic attack (TIA) • diabetes • a neurological condition, eg multiple sclerosis (MS), cerebral palsy or learning disability • a problem with your spleen, eg sickle cell disease, or you have had your spleen removed • are seriously overweight.

Who to call for urgent medical problem when we are closed
If you have an URGENT medical problem on a weekday between 8am and 6.30pm when the surgery is closed, please call 020 8865 0350. If you need assistance after 6.30pm, or on a weekend or Bank Holiday then please call 111.
We aim to provide high quality care in all aspects of medical care to our patients and encourage our patients to take an active care in their health. We are a fully computerised practice and all consultations, medical histories and test results are recorded in a patient’s computer record.
We respectfully ask patients to refrain from using mobile phones in the Practice and smoking is prohibited in the surgery premises.
Our surgery is accessible to patients using a wheelchair. We have a designated disabled parking space in our car park. We also have a disabled toilet. An interpreter service is available for patients who need it. Patients wishing to use this service will need to book appointments in advance (minimum 1 week ahead) in order for us to organise an interpreter to be present at the time of the appointment. We cannot guarantee that interpreter will always be available for the appointment.
Named Accountable GP
At Holly Park Clinic your named accountable GP is Dr Raithatha. Dr Raithatha will take lead responsibility for the coordination of all services and ensure that they are delivered to the patients based on his clinical judgement. However, it does not mean that he is responsible to see you when you book an appointment. You will be seen by either Dr Raithatha or our salaried doctor or locums.

You have a right to know what information we hold about you. If you would like to see your records please contact the Practice for details on how to go about. Please note that there is a charge to access medical records.
The practice will send out automated reminders regarding pre-booked appointments via your mobile telephone so please keep us informed of your latest mobile number.
